- Any company specific paperwork
- Completed employer authorization form (add hyperlink), signed by your designated company representative
- A photo ID (driver’s license, passport, military ID, employment badge or student identification card)
In lieu of completing this form, the designated company representative may opt to call WORKNET directly and provide employee/employer information and authorization.
If you are responsible for payment, please bring exact amount as we have limited ability to make change for large denominations.

Yes. Your parent/legal guardian should accompany you to your visit. If they are unable to attend, they must complete an “Authorization and Consent on Behalf of a Minor” form that you will need to bring to your visit. Cick here to access the Minor Consent Form.

Yes, each of our facilities has a provider that is National Registry of Certified Medical Examiners-certified and able to conduct DOT physical exams.

Drug test results typically take 24-48 hours, depending on the type of screening performed, as directed by your employer. Alcohol results are given at the time of testing.
